Plan Details:
Welcome to The Dental Studio of Southern Illinois’ in house membership plan! This plan is designed to help patients achieve their oral health goals without the burdens of dental insurance. We hope you utilize this plan for you and your family for years to come. There are many benefits to our membership plan including:
- No annual maximums
- No waiting period – start your treatment today!
- No deductibles
- No ID card necessary
- Cosmetic dentistry included!
Annual Fees & Benefits:
Individual $50
Family Up to Four $100
Family Up to Six $150
Eligibility:
- Must be 18 years or older to be an independent member
- Spouse/domestic partner and dependents can be added up to age 26
- Can be combined with HSA/FSA accounts
- Cannot be combined with insurance plans
Payments:
- Enrollment fee must be paid in full to receive discounts and is non-refundable
- All payments are due in full at the time of service
- Any services not paid in full will be billed at our normal fees
- Your effective date is the day you sign up and your renewal will be the same date every year
Exclusions:
- Plan fees are subject to change yearly
- Treatment initiated prior to enrollment is not eligible
- All orthodontic and bleaching treatments are excluded
- Treatment involving a specialist is not eligible for the discount program and is at the discretion of the doctor
- Treatment fees guaranteed for 6 months
- Two no-shows or cancellations without a 24 hours notice can lead to being dropped from the plan without a refund
- The Dental Studio of Southern Illinois reserves the right to discontinue the plan for any member at anytime
